Rollin J. (Terry) Fairbanks, M.D., M.S., FACEP, is Director of the National Center for Human Factors Engineering in Health Care, which is part of the MedStar Institute for Innovation in Washington, D.C., and affiliated with the Georgetown University Department of Emergency Medicine. He is also an adjunct faculty member in the Department of Industrial Systems Engineering at the University at Buffalo.
Dr. Fairbanks received his Doctor of Medicine degree from the Medical College of Virginia in Richmond, Virginia. In addition, he received a Master of Science degree in engineering from the Virginia Polytech Institute and State University in Blacksburg, Virginia, and a Bachelor of Arts in mathematics and physics from the State University of New York at Potsdam. He also completed a residency in emergency medicine at Strong Memorial Hospital in Rochester, New York.
Dr. Fairbanks is a practicing emergency physician and is a trained safety scientist with a background in human factors engineering and system safety engineering. His research has involved applying human factors engineering, usability, and cognitive engineering methodologies in health care with a focus on engineering systems to improve reliability and resilience. He is a 2008 recipient of the Society for Academic Emergency Medicine's Young Investigator Award and a graduate of the HRET/NPSF Patient Safety Leadership Fellowship. His research has been funded by AHRQ, NINR, NIBIB, and several foundations, including the American Society of Health-System Pharmacists (ASHP) Research and Education Foundation. He has authored more than 58 published papers on patient safety and safety engineering topics. Dr. Fairbanks gives frequent presentations on patient safety and safety engineering topics at national and international meetings.
Most recently, Dr. Fairbanks was at the University of Rochester where his patient safety research program included an AHRQ- supported study on the benefits of using clinical pharmacists in the emergency department (see www.EmergencyPharmacist.org for resources and publications). He is currently in his fourth year serving as a mentor in the ASHP Patient Care Impact Program, which has helped 50 hospitals start clinical pharmacy programs in the emergency department, and he has served as a research mentor for three research projects supported by the ASHP Foundation.

James A. Jorgenson, M.S., B.S.Pharm., FASHP, is Chief Pharmacy Officer and Vice President
at Clarian Health in Indianapolis, Indiana. Mr. Jorgenson is responsible for the direction and
leadership for all Clarian pharmacy programs, including inpatient services that provide direct
pharmacy care for 19 hospitals statewide and ambulatory programs that oversee 19 retail
pharmacies, a mail order pharmacy, and a variety of ambulatory clinical pharmacy services.
Mr. Jorgenson earned a Bachelor of Science degree in pharmacy and a Master of Science
degree in hospital pharmacy from the University of Minnesota. He also completed a two-year
residency in hospital pharmacy administration at United and Children's Hospital in St. Paul,
Minnesota.
Mr. Jorgenson has served on editorial and advisory boards for health care providers and
industry. He has also been an active member of ASHP, serving on the ASHP Goals
Committee, the Commission on Credentialing, and the Councils for Legal and Public Affairs and
Administrative Affairs. He has also held numerous state affiliated chapter positions. Mr.
Jorgenson has authored more than 30 publications and has provided more than 100 invited
presentations both nationally and internationally.
Before joining Clarian, Mr. Jorgenson was Administrative Director for Pharmacy and Associate
Dean for Pharmacy at the University of Utah. He was also responsible for the operation of Drug
Information Services supporting the 2002 Winter Olympic Games, as well as the Polyclinic
Pharmacy providing pharmacy services to the athletes and Olympic family.

James M. Hoffman, Pharm.D., M.S., BCPS, is Medication Outcomes and Safety Officer in the
Pharmaceutical Department at St. Jude Children’s Research Hospital in Memphis, Tennessee.
In his position, Dr. Hoffman leads medication use policy, medication safety, and research
pharmacy services at St. Jude. Dr. Hoffman is also an assistant professor of clinical pharmacy
at the University of Tennessee Health Science Center.
Dr. Hoffman received both his Bachelor of Science degree in pharmacy and Doctor of
Pharmacy degree from the Philadelphia College of Pharmacy. In addition, he received a Master
of Science degree in pharmacy administration from the University of Wisconsin-Madison. He
also completed a residency in pharmacy administration and a fellowship in outcomes research
at the University of Wisconsin Hospital and Clinics.
Dr. Hoffman is a board-certified pharmacotherapy specialist (BCPS). He is an active member of
American Society of Health-System Pharmacists (ASHP), serving on the editorial board of the
American Journal of Health-System Pharmacy (AJHP) and on the Council on Pharmacy
Practice. He has experience analyzing various aspects of national pharmaceutical use and
policy. Since 2004, Dr. Hoffman has been the lead author of an annual AJHP publication on
prescription drug expenditures. He has made local and national presentations on several topics,
including medication safety, medication expenditure patterns, and biosimilars.

Kevin Colgan, M.A., B.S.Pharm., FASHP, is Corporate Director of Pharmacy at Rush University Medical Center in Chicago, Illinois. He has more than 20 years of experience in clinical research and pharmacy practice management in hospital, home care, and retail settings. Mr. Colgan earned a Masters of Arts degree in health services administration from the University of Illinois and a Bachelor of Science degree in pharmacy from St. Louis College of Pharmacy. He is currently a preceptor at Midwestern University College of Pharmacy. Mr. Colgan is Past President of the American Society of Health-System Pharmacists (ASHP). In 2007, he was named a fellow of ASHP. In addition, he received the St. Louis College of Pharmacy Outstanding Achievement Award in 2004, the Hospital Pharmacist of the Year Award from the Illinois Council of Hospital Pharmacists in 1993, and the Presidential Award for Management Excellence in 1987 from Elmhurst Memorial Hospital. Mr. Colgan is one of the founding members of the Pharmacy Technician Certification Board, which has certified over 500,000 pharmacy technicians in the past two decades.

Steve Rough, M.S., B.S.Pharm., is Director of Pharmacy at the University of Wisconsin Hospital
and Clinics and Clinical Assistant Professor at the UW-Madison School of Pharmacy. He is also
Director of the two-year Health-System Pharmacy Administration Residency Training and
Masters Program at UW Hospital.
Mr. Rough received both his Bachelor of Science degree in pharmacy and Master of Science
degree in health-system pharmacy administration from the School of Pharmacy at the University
of Wisconsin. He also completed the residency training program in Pharmacy Practice and
Health-System Pharmacy Administration at the University of Wisconsin Hospital and Clinics.
Mr. Rough has coordinated and implemented progressive pharmacy services in virtually all
practice settings. He has contributed six textbook chapters and numerous other papers in the
pharmacy literature related to pharmacy administration and leadership, medication safety, the
application of automation and technology, operational benchmarking, and a variety of other
management-related topics. He has been an invited lecturer for local, state, and national
audiences.
Mr. Rough recently completed a four-year elected term as Treasurer of the Pharmacy Society of
Wisconsin, and he also currently serves as a member of the UHC Pharmacy Council Executive
Committee. He has served as Chair for the ASHP Section of Pharmacy Practice Managers in
2008, is a member of the ASHP House of Delegates, and has served as Chair for the ASHP
Council on Pharmacy Management. In 2003, Mr. Rough received both the Wisconsin
Pharmacist of the Year award and an ASHP Best Practices Award for the paper he submitted
describing the impact of point-of-care bar code medication scanning technology on medication
error reduction. In 2006, he accepted the inaugural ASHP Foundation Pharmacy Residency
Program Excellence Award on behalf of the University of Wisconsin Hospital and Clinics
administrative residency and masters program. In July 2009, he was awarded the UW Hospital
and Clinics Presidential Leadership Award provided annually to a department director for
exceptional sustained leadership across the organization and in the community.
Mr. Rough is co-creator of the ASHP Managers’ Boot Camp and the ASHP State Affiliate
Student Leadership Development Workshop. His practice and research interests include
pharmacy administrative practice, medication-use safety, work redesign, pharmacy technology,
financial management, and implementation of progressive pharmacy services.
