Susan Cornell, Pharm.D., CDE, FAPhA, FAADE, is Assistant Director of Experiential Education and Assistant Professor, Department of Pharmacy Practice at Midwestern University Chicago College of Pharmacy in Downers Grove, Illinois. Dr. Cornell is a certified diabetes educator and clinical pharmacy consultant, specializing in community and ambulatory care practice.
Dr. Cornell has over 18 years of practice in community pharmacy where she is a clinical pharmacist, diabetes educator and preceptor, as well as, the coordinator of Dominick's Pharmacy Diabetes Self-Management Education program, which received American Diabetes Association (ADA) recognition in December of 2001. Her current clinical practice is at the Access DuPage Community Clinic in Wheaton, Illinois, where she trains and supervises students from the colleges of medicine, health sciences, and
pharmacy, as they provide diabetes education classes for underserved patients.
Dr. Cornell received her Bachelor of Pharmacy degree from the University of Illinois, College of
Pharmacy in 1986 and her Doctor of Pharmacy degree from Midwestern University in May of
2002. Dr. Cornell is a Fellow of the American Pharmacists Association (APhA) and a 2008 Fellow of the American Association of Diabetes Educators (AADE). She has received many professional awards, including the 2006 Alumnus of the Year Award from Midwestern University Chicago College of Pharmacy, the 2005 Midwestern University Golden Apple Teaching Award and the 2003 Illinois Pharmacists Association, Pharmacist of the Year award. In addition, Dr. Cornell is an active member of ADA and AADE, where she served on the board of directors from 2004 to 2007. She is a faculty advisor for the Midwestern University chapter of APhA-Academy of Student Pharmacists (APhA-ASP) Operation Diabetes and Collaborative Health Advocate Team project and serves on many committees and advisory groups for other professional organizations.
Dr. Cornell has given presentations on diabetes and its related conditions nationally and internationally for the past ten years and is recognized as a key opinion leader in the field of diabetes education. She has contributed to and has been featured in numerous publications.

Debra A. Goff, Pharm.D., FCCP, is Infectious Diseases Specialist and Antibiotic Utilization Review Coordinator at The Ohio State University Medical Center (OSUMC) in Columbus, Ohio. Dr. Goff is also Clinical Associate Professor at The Ohio State University College of Pharmacy.
Dr. Goff serves as a preceptor in the infectious diseases residency program at OSUMC and she is Chairperson for the OSUMC Robert J. Fass Memorial Infectious Diseases Fund. She received her Bachelor of Science and Doctor of Pharmacy degrees and then completed a pharmacy residency at the University of Illinois at Chicago.
Dr. Goff serves as an abstract reviewer for the American College of Clinical Pharmacy. She was honored by the OSUMC Leadership Council for Clinical Value Enhancement for developing clinical practice guidelines entitled "Community-Acquired Pneumonia in Immunocompetent Adult Patients." She has received over 100 research grants and presented over 200 lectures nationally and internationally. Dr. Goff has published in several scientific journals, including Pharmacotherapy, Current Opinion in Infectious Diseases, Archives of Internal Medicine, and Clinical Infectious Diseases. Dr. Goff is a Fellow of the American College of Clinical Pharmacy and a member of the Infectious Diseases Society of America, the American Society for Microbiology, and the Society of Infectious Diseases Pharmacists.
Dr. Goff's research interests include antimicrobial resistance, clinical outcomes research, and antifungal agents. During 2009 Dr. Goff traveled to China, Chile, Portugal, and Africa to assist in the development of model antimicrobial stewardship programs.

Michael D. Hogue, Pharm.D., FAPhA, is Director of Experiential Programs and Associate Professor of Pharmacy Practice at Samford University's McWhorter School of Pharmacy in Birmingham, Alabama. Dr. Hogue also serves as ambulatory care and public health specialist with the Jefferson County (Alabama) Department of Health. He has practice experience in managed care, community pharmacy, and home infusion.
Dr. Hogue received his Doctor of Pharmacy degree from Samford University in Birmingham, and then completed an executive management residency with the American Pharmacists Association (APhA) in Washington, DC. He has authored numerous articles and book chapters, including The Pharmacists Guide to Compensation for MTM Services, published by APhA. He represents APhA on the Pharmacy Services Technical Advisory Committee.
Dr. Hogue is one of the lead authors of APhA's Pharmacy-Based Immunization Delivery certificate training program, and has been since its inception in 1996. Dr. Hogue is an active member of several professional pharmacy organizations, including APhA, the American Society of Health-System Pharmacists, and the American Association of Colleges of Pharmacy. He was recently elected as APhA's Academy of Pharmacy Practice and Management President-Elect for 2010-2011.

Joseph Saseen, Pharm.D., FCCP, FNLA, BCPS, CLS, is Professor of Clinical Pharmacy and a clinical pharmacy specialist at the University of Colorado A.F. Williams Family Medicine Center in Denver, Colorado. Dr. Saseen is a board certified pharmacotherapy specialist with added qualifications in cardiology, is certified as a Clinical Lipid Specialist, and is a Fellow of the American College of Clinical Pharmacy.
Dr. Saseen received his Bachelor of Science in Pharmacy and Doctor of Pharmacy degrees from the State University of New York at Buffalo and then completed a fellowship in ambulatory care research at the University of Illinois at Chicago. In his current position, Dr. Saseen is chairman of the curriculum committee at the University of Colorado School of Pharmacy. He serves on the Board of Directors of the Southwest Lipid Association. His professional efforts include national recognition of pharmacists as healthcare providers, post-graduate pharmacy residency training, and specialty board certification.
Dr. Saseen participates in research related to hypertension, dyslipidemia, and rheumatoid arthritis. He has published pharmacotherapy review articles and textbook chapters related to the management of cardiovascular diseases. Dr. Saseen is a member of the Board of Directors for the National Lipid Association's Accreditation Council for Clinical Lipidology. He has received several teaching awards, and most recently was the recipient of the University of Colorado Denver School of Pharmacy Doctor of Pharmacy Class of 2011 Outstanding Instructor Award.

Curtis L. Triplitt, Pharm.D., CDE, is Clinical Specialist and Certified Diabetes Educator at the Texas Diabetes Institute in San Antonio, Texas, where, together with an endocrinologist, he clinically manages and educates people with diabetes. Dr. Triplitt also is Clinical Assistant Professor of Medicine in the Division of Diabetes at the University of Texas Health Science Center at San Antonio (UTHSC-SA). In addition, he is Clinical Assistant Professor of Pharmacy in the clinical pharmacy program at UTHSCSA, and serves as a preceptor for Doctor of Pharmacy students and residents.
Dr. Triplitt earned his Bachelor of Science in Pharmacy degree from the University of Iowa and his Doctor of Pharmacy degree from the University of Texas at Austin and the Health Science Center at San Antonio. He completed an ASHP-accredited residency in primary care at the William S. Middleton Veteran Administration's Hospital in Madison, Wisconsin.
Dr. Triplitt is well respected as a clinician, researcher, and author. He is an investigator in several ongoing research studies related to diabetes, and he has published several book chapters on diabetes, as well as articles in peer-reviewed journals, including Diabetes Care, Diabetes Spectrum, Expert Review of Endocrinology & Metabolism, Pharmacotherapy, and Drugs. Dr. Triplitt also serves as the interim chair of the Medical Professionals Advisory Subcommittee of the Texas Diabetes Council, which is legislatively mandated to develop and implement a state plan for diabetes treatment, education, and training. The subcommittee's mission is to develop standards of care for the prevention, identification, and treatment of patients with diabetes mellitus in Texas and to develop and update diabetes treatment algorithms.
