This Award program is a national competition that recognizes
innovation and outstanding practitioners in health-system
pharmacy. Recipients of the ASHP Best Practices Award in Health-System
Pharmacy will present posters of the successful implementation
of an innovative program which improved the quality of patient
care in their health system at the ASHP Midyear Clinical Meeting.
The ASHP Best Practices Award in Health-System Pharmacy was
established in 1999.
The goal of the award program is to recognize and foster innovation
in health-system pharmacy.
There are many outstanding practices that deserve recognition.
Applicants will be judged on their descriptions of programs
and practices currently employed in their health system based
on the following criteria:

Originality and innovative nature of the program,
-
Significance of the program to the health system,
-
Demonstration of improvements in quality of patient
care,
-
Significance of the program to pharmacy practice, and
-
Quality of the descriptive report.
As an Award recipient the program will receive national attention.
Other health-system clinical and administrative pharmacists
may even adopt the program or develop their own creative clinical or managerial initiatives based on the program.
Eligibility
Applicants must be ASHP members practicing in a health-system
setting such as:*

Ambulatory care clinic
-
Chronic care
-
Home health care
-
Inpatient care
-
Outpatient pharmacy
More than one successful program from a health system may
be submitted for consideration.
* Persons employed by Amgen are not eligible to participate in this award program.
Award

A maximum of six health systems will be recognized.

The award recipient will receive a plaque and national recognition.
-
Award recipients will present a poster of their program
at a reception sponsored by Amgen during the ASHP Midyear
Clinical Meeting in December. Award recipients are responsible for travel
expenses to present their poster at the Midyear Clinical Meeting.
